The Intake Form is the foundation for building the binder for your entire guardianship application — from fee waiver and deferral request to the petition, and other requirements. The Intake form is online and can be completed in 10-15 minutes if you have all the petitioner’s, guardian(s), and protected person’s demographic, contact, and benefit information. An initial payment is required to begin the Intake process and before DIY Guardianship begins building your binder of guardianship application forms.

After the Intake form is completed and an initial payment is made, DIY Guardianship will complete 95% of the guardianship forms, provide the remaining 1-2 forms to you with clear instructions for your completion, and arrange to meet you at court to file the forms.
